Lip flip and lip filler are often discussed together, and patients frequently arrive unsure which one they actually need. The distinction is simple once you understand what each one does — and the difference matters significantly for choosing the right treatment for your goals.
A lip flip uses a few small units of Botox placed around the orbicularis oris — the circular muscle surrounding the mouth. By relaxing the portion of this muscle that pulls the upper lip inward, the lip gently rolls outward (everts). The result is an upper lip that looks a little fuller and more visible when you smile or speak — without any volume actually being added.
The effect is subtle and soft. The upper lip does not get bigger — it simply shows more of itself. Patients who feel their upper lip 'disappears' when they smile, or who want more upper lip visible at rest, often respond extremely well to a lip flip.
Lip filler — typically hyaluronic acid (HA) injected into the lip tissue — adds actual volume. It makes the lips physically larger and can add structure, definition to the cupid's bow, and vertical height. If someone wants noticeably fuller or more structured lips, filler is the appropriate treatment. A lip flip cannot achieve that level of change — it enhances shape and visibility, not volume.
The treatment itself takes about 5 minutes. Most patients describe the injections as a minor pinch. After treatment, the lip feels completely normal — no numbness, no significant swelling. Results appear within 5 to 7 days as the orbicularis muscle relaxes and the upper lip gently rolls outward.
Duration is one of the most important practical differences between the two treatments. A lip flip typically lasts 6 to 8 weeks — shorter than other neurotoxin treatments because the orbicularis oris is a highly active muscle used constantly in speaking, eating, and expression. Lip filler, by contrast, typically lasts 6 to 12 months. The lip flip is a much lower commitment option — ideal if you want to try lip enhancement without a long-lasting change.
